The role: We are looking for a full time Social Team Leader - Residential to guide and inspire ACH Group’s Social Residential team in the delivery of a sustainable, high-impact social programs across our residential care homes This role is an opportunity to lead transformative work that fosters community, restores valued roles, and directly combats social isolation.
What you’ll do:
- Mentor and lead a dedicated team to deliver best-practice social initiatives within residential aged care settings.
- Oversee the implementation of a sustainable core social program that supports community engagement and personal growth for residents.
- Support residents to reconnect with their sense of purpose, rediscover valued roles, and maintain or establish meaningful community connections.
- Champion efforts to challenge age-related stereotypes and promote a strengths-based view of ageing.
- Evaluate and report on program outcomes to ensure continuous improvement and long-term sustainability.
- Work cross-functionally with care teams, volunteers, families, and the broader community to embed social wellbeing into daily living.
About you!
- Qualifications in health, community, business or other relevant area or working towards.
- Demonstrated people leadership skills.
- Experience in the coordination of small to medium sized events.
- Aged Care or disability experience.
- Understanding of the Aged Care Standards, various government funding and private options that support social programs.
- Proven experience in community development, social programs, or aged care leadership.
- A passion for social inclusion, connection, and reimagining what ageing can look like.
- Strong leadership and people development skills with a collaborative mindset.
- Strategic thinking and the ability to embed programs into daily practice.
- Excellent communication and stakeholder engagement abilities, to work with volunteer, staff and customers.
- A Police Check for working in Aged Care less than 12 months old at commencement.
- A current drivers licence.
