As the Level 3 Occupational Therapist in the transition team you will be expected to plan, coordinate, and implement in-depth levels of clinical expertise and extensive specialised knowledge within Occupational Therapy in order to provide high quality care to patients with complex care needs within or accessing St George Hospital, consistent with NSW Health, South Eastern Sydney Local Health District (SESLHD) policies, procedures and standards. This position provides assessment, intervention and care coordination to patients associated with St George Hospital. The role of the Transition Team is to ensure processes and clear timeframes are in place for discharge planning as patients' transition through each phase of their care. Target patient groups include complex and vulnerable inpatients of St George Hospital, patients requiring NCAT for discharge or where concerns around capacity and decision making are identified, and patients on an NDIS pathway to transition into the community. The role is also focused on capacity building and supporting allied health, nursing and medical staff to provide excellent care to people with a disability and healthcare needs while they are in hospital. You will also have opportunity to influence emerging models of care related to hospital avoidance and the interface between community and inpatient care, and to contribute towards a wide range of disability related initiatives. The role will require flexibility and adaptability in line with the changing needs of our services as the role continues to evolve within the organisation.

  1. Qualifications in accordance with Schedule C of the NSW Health Service Health Professionals (State) Award and Current Registration with AHPRA.
  2. Extensive post graduate clinical experience in an inpatient setting managing patients with complex care needs.
  3. Excellent negotiation, decision making and advocacy skills including the ability to interact constructively and collaboratively with a diverse range of stakeholders, throughout all organisational levels.
  4. Ability to provide in-depth advice and education to enhance service delivery and to guide and support others in providing service excellence, with the capacity to provide clinical supervision to other staff.
  5. Ability to work as part of a multi-disciplinary team and to apply advanced reasoning skills and independent professional judgement when dealing with situations of a novel, complex or critical nature.
  6. Ability to initiate, lead, complete and share quality improvement initiatives and service evaluation processes and lead ongoing quality improvement activities with other staff.
  7. Demonstrated commitment to ongoing clinical and professional learning.
  8. Current class C drivers licence.
