Roles and responsibilities:
In this role, you will provide high-level, person-centered support to individuals with significant physical, intellectual, or psychosocial disabilities—many of whom present with complex behavioral needs. You will be part of a multidisciplinary team committed to delivering safe, respectful, and empowering care that promotes independence and quality of life in the community and in home support required.
Key Responsibilities
- Deliver intensive, Individualised support to clients with high and complex needs, including behaviors of concern
- Implement and adhere to Positive Behavior Support (PBS) plans and restrictive practice protocols
- Provide personal care, mobility assistance, medication administration, and support with daily living tasks
- Respond calmly and effectively to behavioral escalations using trauma-informed and de-escalation techniques
- Maintain accurate, timely documentation and incident reporting
- Collaborate with families, allied health professionals, and internal teams to ensure holistic care
- Contribute to a safe, inclusive, and culturally sensitive environment
- Essential Criteria
- Minimum 4+ years’ experience working with participants with complex behaviors and high support needs
- Physco- social / mental health
- Certificate IV in Disability, Individual Support, or equivalent
- Demonstrated ability to work effectively in highly dynamic, fast-paced, and challenging environments demonstrating exceptional time management and documentation skills
- Strong understanding of Positive Behavior Support and trauma-informed care principles
- Excellent communication, problem-solving, and conflict resolution skills
- Current First Aid & CPR, NDIS Worker Screening Check
- Manual handling, medication administration, and infection control training essential
- Experience with complex bowel and associated problems
- Valid driver’s license and reliable transport (if community access is required)
- NDIS Screening check
- Valid work visa holders with full work rights
