We are seeking a qualified and experienced Community Worker to join our dynamic team. The purpose of this role is to provide case-based support to clients with diverse social, emotional, and practical needs. As a Community Worker, you will advocate for client wellbeing, facilitate access to essential services, and develop intervention strategies that promote independence and social inclusion. This role requires compassion, initiative, and strong knowledge of community-based support systems.

You will work closely with clients to assess their needs, provide ongoing support, coordinate resources, and ensure they are linked to the services they require for sustainable well-being. The role suits someone passionate about making a difference in the lives of people experiencing hardship or marginalisation.

Task & responsibilities

  • Provide person-centred, culturally appropriate support to individuals and families in need
  • Conduct community outreach and needs assessments
  • Facilitate access to social services, housing, healthcare, and other support programs
  • Develop and implement individualised support and care plans
  • Advocate on behalf of clients to ensure access to entitlements and services
  • Build partnerships with local organisations and community networks
  • Monitor client progress and maintain accurate case notes and reports
  • Promote community inclusion and capacity building through education and empowerment

Qualification & experience

  • A diploma or higher qualification in Community Services, Social Work, Welfare, or a related field.
  • Minimum 1 year of relevant experience in community services or a similar role.
  • Experience working with vulnerable or marginalised groups, including individuals with disability, mental health concerns, or from culturally diverse backgrounds.
  • Understanding of relevant legislation, policies, and practices in the Australian community services sector.
  • Strong interpersonal and communication skills (written and verbal).
  • Ability to work independently and manage complex client needs.
  • High level of cultural competence and sensitivity.
  • Proficiency in report writing and client record management.
  • Empathetic, resilient, and solutions-focused approach.
  • Fluent in English; additional languages are a plus.
