This excellent opportunity will be offered on a Permanent Full-Time basis at 38 hours a week.

Task & responsibilities

  • Assess clients’ individual needs and implement person-centered support plans to enhance independence, wellbeing, and quality of life. 
  • Provide daily care and supervision to participants in a residential/group home setting, including assistance with personal hygiene, mobility, and routine activities. 
  • Develop, implement, and maintain structured routines and skill-building activities tailored to each participant’s needs, preferences, and goals. 
  • Monitor clients’ physical, emotional, and behavioral conditions, and accurately record and report progress, incidents, and changes in circumstances to management. 
  • Observe and assess client behaviors and responses to identify risks, concerns, or changes, and respond appropriately in accordance with organisational policies and care plans. 
  • Support clients in building social connections, participating in community activities, and improving their overall health and wellbeing. 
  • Liaise and collaborate with families, stakeholders, support coordinators, healthcare professionals, and internal teams to ensure coordinated and effective service delivery. 
  • Facilitate communication between clients and relevant stakeholders, ensuring clients’ voices, choices, and rights are respected and upheld. 
  • Assist in the admission and transition of clients within the residential service, ensuring continuity of care and smooth integration into the environment. 
  • Maintain accurate documentation, including daily progress notes, incident reports, care records, and compliance documentation in line with organisational and regulatory requirements. 
  • Identify and report risks, hazards, and safeguarding concerns, including suspected abuse, neglect, or exploitation, and take appropriate action in line with duty of care obligations. 
  • Support the implementation of health and safety practices, including infection control, manual handling, and risk management procedures. 
  • Promote a safe, inclusive, and supportive environment that respects diversity, cultural background, and individual rights of all participants. 
  • Coordinate with team members to ensure effective shift operations, continuity of care, and adherence to service standards and organisational policies. 
  • Participate in training, team meetings, and continuous professional development activities to maintain high standards of care and compliance with NDIS requirements.

Qualification & experience

  • Hold a relevant AQF associate degree or equivalent, or higher qualification. You can also have three years of relevant work experience or part-time equivalence in lieu of the qualification.
  • Current First Aid certificate and CPR
  • National Police Check
  • Minimum of 1 year of work experience
  • The right to work in Australia 
  • Ability to work in a fast-paced environment
  • Strong time management skills
