The Residential Care Officer at Cosmos Care Group is responsible for delivering daily care and support to residents, including personal care assistance, mobility support, meal assistance, monitoring well-being, implementing individual care plans, maintaining accurate care documentation, and collaborating with healthcare professionals and family members. The role also includes encouraging social participation, assisting with medication supervision where authorised, supporting new residents during orientation, maintaining workplace health and safety standards, identifying potential risks, and participating in team meetings and ongoing training to ensure high-quality care and service delivery.
DUTIES & RESPONSIBILITIES
• Ensure the safety, well-being, and dignity of residents with disabilities in a residential care setting.
• Assess individual needs of NDIS participants to develop and implement personalised support, training, and life skills programs.
• Support and empower residents to build independence, enhance daily living skills, and improve overall quality of life.
• Facilitate and encourage resident participation in community activities to promote inclusion and social engagement.
• Conduct client interviews to identify personal, emotional, and social challenges impacting their well-being.
• Monitor and record resident progress, updating support plans as needed and maintaining accurate documentation.
• Identify local resources and services related to health, housing, employment, and other areas relevant to NDIS participants.
• Deliver structured skill-building and activity programs with limited supervision, both individually and as part of a team.
• Liaise with families, support coordinators, community groups, service providers, and government agencies to ensure coordinated care.
• Provide care and support in group homes and other supported accommodation settings, assisting with daily routines and personal tasks.
• Assist with meal planning, preparation, and cooking according to resident needs and dietary requirements.
• Help residents manage behavioural challenges by implementing Behaviour Support and Management Plans.
• Refer participants to relevant allied health professionals, services, or community supports as appropriate.
• Advocate for residents' rights and preferences in all aspects of care and community engagement.
• Prepare reports, care documentation, and funding submissions in line with NDIS compliance and service requirements.
• Respond to general enquiries, assist with special projects, and contribute to a safe, supportive, and person-centred environment.
QUALIFICATIONS & REQUIREMENTS
• AQF associate degree, Advanced Diploma or Diploma (ANZSCO Skill Level 2)
• 1 year of relevant work experience required for this role
• Strong interpersonal and communication skills
• Ability to work independently and without supervision.
• Understanding of duty of care and person-centred approaches
WORKING HOURS: Full-time position, 38 hours per week.
