Job Description

Reliance Community Services Ltd is seeking compassionate and reliable Support Workers to provide in-home support and community access assistance to an individual living with disability in the Grafton/Waterview Heights region.

This role focuses on empowering the participant to live safely and independently while promoting community participation, social connection, routine, and quality of life.

Key Responsibilities

• Provide person-centred support in line with the participant’s support plan

• Assist with daily living activities, including prompting and support with personal care and self-care routines

• Support community access, including appointments, shopping, social outings, fishing, gym visits, and recreational activities

• Assist with meal preparation, household tasks, and maintaining a safe home environment

• Encourage independence, routine, and social participation

• Ensure the participant’s safety and wellbeing at all times

• Maintain accurate progress notes and documentation

• Communicate effectively with the Team Leader and relevant stakeholders

About You

• Patient, empathetic, and respectful

• Strong communication and interpersonal skills

• Able to work independently and use initiative

• Comfortable supporting individuals with psychosocial disability, dementia, or cognitive impairment

• Committed to upholding participant dignity, rights, and choice

• Minimum of 1+ years experience in a support role

Essential Requirements

• Current NSW Working With Children Check (or willingness to obtain)

• NDIS Worker Screening Check

• First Aid & CPR Certificate

• Full Australian Driver’s Licence

• Registered and comprehensively insured vehicle

• Legal right to work in Australia

• Certificate III in Individual Support or equivalent qualification preferred

This is a casual position under the SCHADS Award with flexible weekday and weekend shifts available.
