Are you driven to support and empower others? Care to Heal Pty Ltd is looking for qualified, experienced, caring, dependable, and motivated Residential Care Officer/Care Corodinator to join our healthcare team.

Key Responsibilities

In this role, you will:

  • Assessing clients' needs and planning, developing and implementing educational, training and support programs
  • Interviewing clients and assessing the nature and extent of difficulties
  • Supervising and assisting residents with daily living activities, including personal care, hygiene, meals, and medication (as directed)
  • Managing routines, appointments, activities, transport and contending responsibilities
  • Implementing and monitoring individual care and behaviour support plans
  • Recording daily progress notes, tracking behaviour goals, and maintaining compliance documents
  • Communicating updates and coordinating care with family members, caseworkers, and health professionals
  • Promoting independence, participation, and social inclusion of residents
  • Providing respectful support to people from diverse cultural and linguistic backgrounds and appropriately handling personal and sensitive information
  • Identifying risks, following procedures and responding appropriately to incidents
  • Responding appropriately to changing circumstances while following organizational policies and individual care plans
  • Referring clients to agencies that can provide additional help
  • Ensuring compliance with NDIS Quality and Safeguards Commission standards
  • Monitoring and reporting on the progress of clients
  • Assessing community need and resources for health, welfare, housing, employment, training and other facilities and services
  • Liaising with community groups, welfare agencies, government bodies and private businesses about community issues and promoting awareness of community resources and services
  • Supporting families and providing education and care for children and disabled persons in adult service units, group housing and government institutions
  • Supervising offenders on probation and parole (if required)
  • Assisting young people to solve social, emotional and financial problems
  • Preparing paperwork, plans reports for relevant assistance to government bodies and other agencies

 

Qualifications, Compliancy & Experience Requirements:

  • Diploma/or higher qualification in Community Services/Disability/Welfare
  • Atleast One Year or higher work experience as Residential Care Officer/Care Coordinator in healthcare sector
  • Current First Aid & CPR certification
  • NDIS Worker Screening Check
  • National Police Check (within 12 months)
  • Evidence of right to work in Australia
  • Current vaccinations as required for NDIS, community and aged care work
  • Demonstrated knowledge of NDIS Quality and Safeguards Commission standards and incident reporting.
  • Valid Australian Driver Licence & reliable vehicle with appropriate insurance
  • Ability to work independently in community settings
  • Strong communication, documentation & clinical observation skills
