About the Role

We are seeking an experienced and compassionate Support Coordinator/Community Worker to join our team. The successful candidate will work closely with NDIS participants, families, carers, service providers and community organisations to understand individual needs, coordinate appropriate supports and assist participants to achieve their NDIS and personal goals.

Key Responsibilities

• Assess the needs and support requirements of NDIS participants.
• Interview participants to understand their needs, goals and personal circumstances.
• Develop and coordinate support activities based on participants’ needs, goals and interests.
• Monitor participants’ progress and maintain records and reports.
• Refer participants to suitable health, welfare, community and other support services.
• Help participants access community services, programs, resources and support.
• Liaise with families, carers, community organisations, welfare agencies and service providers.
• Provide information and guidance about available community services and resources.
• Support participants to develop life skills, confidence, independence and social connections.
• Assist participants to overcome barriers to community participation.
• Encourage and support participation in social, recreational and community activities.
• Advocate for participants and help them access suitable services and opportunities.
• Maintain accurate participant records, case notes and progress reports.
• Maintain confidentiality and follow NDIS, workplace health and safety and organisational requirements.

Skills and Experience

• Relevant Diploma or higher qualification in Community Services, Welfare, Social Services or a related field.
• Minimum 1–2 years of relevant experience in community services, welfare or support services.
• Experience assessing individual support needs and identifying suitable services and resources.
• Knowledge of community, welfare and social support services.
• Strong communication, interpersonal and relationship-building skills.
• Good organisational, coordination and record-keeping skills.
• Ability to work effectively with participants, families, community organisations and service providers.
• Ability to work independently and as part of a team.
• Understanding of confidentiality, person-centred support and community participation.
• Empathy, patience and a genuine commitment to supporting the wellbeing and independence of people with disability.
