The Community Worker supports individuals with disability and other support needs to improve their independence, wellbeing, social participation and access to appropriate community services. The role provides person-centred support, assists participants to achieve their goals, advocates for their needs and connects them with relevant services and community resources.
The Community Worker provides practical and person-centred support to individuals, assisting them to develop independence, participate in community activities and access appropriate community services and resources. The role works closely with individuals, families, carers, support providers and other relevant stakeholders to understand individual needs, monitor progress and promote wellbeing, social inclusion and meaningful participation within the community.
DUTIES & RESPONSIBILITIES
• Assess participants' individual needs, circumstances, goals and support requirements and develop appropriate person-centred support approaches.
• Provide practical assistance and guidance to participants to promote independence, confidence, wellbeing and participation in daily and community activities.
• Support participants to access community services, health services, education, employment opportunities, recreational activities and other appropriate resources.
• Assist participants to identify personal goals and develop strategies to achieve greater independence and social inclusion.
• Advocate for participants' needs, preferences, rights and interests when communicating with service providers, families and other relevant stakeholders.
• Establish and maintain positive, respectful and professional relationships with participants, families, carers and community service providers.
• Assist participants with community participation, including attending appointments, social activities, community events and recreational programs where required.
• Provide information and referrals to appropriate government, community, health and social support services based on participants' individual circumstances.
• Monitor participants' progress and provide feedback regarding changes in their needs, circumstances, wellbeing and achievement of goals.
• Maintain accurate and confidential participant records, case notes, progress reports and other documentation in accordance with organisational and NDIS requirements.
• Identify and report concerns relating to participant safety, wellbeing, safeguarding or changes in support requirements to management.
• Assist participants in developing daily living skills, decision-making abilities, communication skills and confidence to increase their independence.
• Coordinate with support workers and other service providers to promote continuity and consistency of participant support.
• Support participants to develop and maintain social connections and meaningful relationships within their communities.
• Participate in case discussions and meetings with management, families, carers and relevant professionals to review participant needs and support outcomes.
• Ensure all support activities are delivered in accordance with organisational policies, participant rights, privacy and confidentiality requirements, and relevant NDIS standards.
QUALIFICATIONS & REQUIREMENTS
• Must have a relevant associate degree, advanced diploma, diploma or higher qualification.
• At least 1 year of full-time relevant experience.
• In some cases, relevant experience and/or on-the-job training may be required in addition to formal qualifications.
• Strong teamwork abilities, communication skills, and capacity to follow instructions accurately.
