The successful applicant will work closely with residents, families, healthcare professionals, and external service providers to support residents' individual needs, wellbeing, independence, and social inclusion.

Task & responsibilities

• Assess residents' care, welfare and support needs and assist in developing individual support plans.
• Implement programs that promote independence, community participation, social engagement and quality of life.
• Interview residents and families to identify support requirements and address personal, social and wellbeing concerns.
• Monitor residents' progress and maintain accurate records and reports.
• Liaise with healthcare providers, allied health professionals, government agencies and community organisations to coordinate support services.
• Refer residents to appropriate health, welfare, housing and community services where required.
• Assist residents with accessing available community resources and support programs.
• Support residents with daily living activities and encourage independent living skills.
• Provide guidance and emotional support to residents and their families.
• Prepare reports, documentation and case notes in accordance with organisational and regulatory requirements.
• Participate in the planning and delivery of leisure, lifestyle and wellbeing activities.
• Ensure services are delivered in accordance with relevant legislation, policies, procedures and quality standards.

Qualification & experience

• Diploma in Community Services, Individual Support, Disability, Aged Care, Social Welfare or higher qualification in a related field.
• Demonstrated experience working within residential care, community services, disability services or aged care environments.
• Strong interpersonal, communication and case management skills.
• Ability to assess client needs and coordinate appropriate support services.
• Experience liaising with professionals, families, community organisations and government agencies.
• Excellent documentation, reporting and record-keeping skills.
• Understanding of person-centred care principles and resident rights.
• Current National Police Check and relevant employment clearances.
