About Company Care Force Pty Ltd is a growing community and disability services provider delivering person-centred support services across Tasmania. We are currently seeking a full-time Community Worker to join our team in Rosny Park, TAS. About the Role
The Community Worker will support individuals with disabilities and complex support needs to improve their social participation, independence, wellbeing, and access to community services. The role involves working collaboratively with clients, families, healthcare professionals, and community stakeholders to deliver person-centred support programs and promote positive client outcomes. Key Responsibilities
- Assisting clients to access community, social, recreational, educational, and vocational activities
- Supporting the implementation of individual support plans and community participation programs
- Promoting client independence, inclusion, wellbeing, and life skill development
- Monitoring client progress and reporting behavioural, emotional, or wellbeing concerns
- Liaising with families, allied health professionals, NDIS representatives, and external service providers
- Assisting with client advocacy and referral to appropriate community services
- Maintaining accurate case notes, incident reports, and client documentation
- Conducting risk assessments and ensuring compliance with NDIS and workplace policies
- Participating in care planning meetings and multidisciplinary team discussions
- Supporting clients in achieving personal and social development goals
Skills and Experience
- Relevant qualification in Community Services, Disability, Social
Work, Mental Health, or related field
- Previous experience within community services, disability services, or client support environments preferred
- Strong interpersonal, communication, and stakeholder engagement skills
- Ability to work collaboratively within a multidisciplinary environment
- Understanding of person-centred support practices and community care principles
- Current
First Aid and CPR certification preferred
- Current driver licence and
