The role involves identifying individual and community support needs, connecting clients with suitable services, coordinating programs, and encouraging social inclusion and independence. The successful candidate will work closely with stakeholders to deliver effective support plans and community-based initiatives.

Task & responsibilities

Assess clients’, residents’, and community members’ social, emotional, cultural, financial, health, housing, employment, and support needs, and contribute to the development and review of individualized care and support plans.  

Interview clients, residents, and family members to identify difficulties, personal goals, and required support services.  

Plan, develop, implement, and evaluate community programs, educational initiatives, training activities, and support services that promote wellbeing and community participation.  

Facilitate community development initiatives and collective solutions to address issues related to recreation, health, housing, employment, and welfare matters.  

Monitor, document, and report on client and participant progress, wellbeing, and program outcomes in accordance with organizational and regulatory requirements.  

Refer clients and families to appropriate health, welfare, housing, disability, employment, and community support agencies for additional assistance.  

Liaise with community groups, welfare agencies, government bodies, private businesses, and service providers to coordinate services and promote awareness of community resources.  

Support families, elderly residents, children, young people, and people with disabilities through advocacy, education, emotional support, and community engagement activities.  

Assist individuals experiencing social isolation, unemployment, illness, crisis situations, substance misuse, emotional distress, or financial difficulties impacting their quality of life.  

Organize and coordinate community events, recreational programs, cultural activities, sporting functions, and group engagement initiatives.  

Prepare reports, case notes, funding submissions, support documentation, and progress reports for internal purposes and external agencies.  

Collaborate with multidisciplinary teams, including nursing, allied health, lifestyle, and welfare professionals, to deliver holistic, person-centred services.  

Ensure all activities comply with organizational policies, privacy requirements, funding guidelines, and relevant industry standards.


Qualification & experience

AQF Diploma, Advanced Diploma, or associate degree in Community Services, Disability, Aged Care, Human Services, or related field.  

 Minimum 1–2 years’ experience as a community Worker.

Strong communication, assessment, documentation, and teamwork skills.  

Knowledge of person-centred care and community support services.  

Current First Aid & CPR Certificate and NDIS Worker Screening Clearance.
