Clinical Care Coordinator / Nurse Advisor (Aged Care)

This role would be ideal for an experienced Clinical Leader looking to step into a highly visible leadership position, mentor clinical staff, and continue building their management experience within a supportive environment.

Who we are looking for

• Provide clinical and operational leadership to deliver safe, high-quality, person-centred care across the service
• Lead the development, implementation, and continuous review of individualised care plans aligned with best-practice clinical standards
• Mentor and support nursing and care teams to strengthen clinical capability, teamwork, and professional development
• Coordinate multidisciplinary engagement with external providers including GPs, allied health professionals, hospitals, and families to ensure holistic care delivery
• Oversee quality, risk, and operational functions including clinical escalation, audits, documentation, workforce coordination, communication processes, and continuous improvement initiatives

Essentials

• Demonstrated experience in a Care Coordinator or similar clinical leadership role, ideally within aged care
• Availability to work permanent Monday to Friday afternoon shifts (2:00pm–10:00pm)
• Current unrestricted Registered Nurse Division 1 registration with AHPRA
• Strong understanding of aged care clinical processes including assessments, care planning, AN-ACC, accreditation standards, and documentation requirements
• Postgraduate qualifications in aged care or related fields highly regarded but not essential
