As a Clinical Care Partner – Support at Home, you’ll work alongside older adults receiving CHSP or Support at Home funding, empowering them to remain connected, confident, and in control of their lives.

You’ll be responsible for:

What you’ll bring:

✔ AHPRA registration as an Enrolled Nurse
✔ Experience in community, aged care or care partnering
✔ Strong understanding of restorative and wellbeing-focused care
✔ Ability to develop meaningful, goal-driven care plans
✔ Excellent relationship-building and communication skills
