Clinical Care Coordinator (APS6) - Mental Health | Veteran Support

The Opportunity

Join a highly respected national organisation delivering critical mental health support services to veterans and their families.

As a Clinical Care Coordinator (APS6), you'll work at the frontline of care-supporting individuals with complex mental health and psychosocial needs and helping them engage meaningfully with the services they need.

This is a hands-on, client-focused role combining clinical intervention, case management, and stakeholder engagement within a supportive multidisciplinary environment.

What You'll Be Doing

• Deliver short-term, goal-focused clinical case management to clients with complex needs
• Provide brief interventions, risk assessments, care planning and referrals
• Support clients to overcome barriers to accessing mental health care
• Work collaboratively with internal teams and external service providers
• Build strong relationships across health, community and government stakeholders
• Maintain accurate client records using case management systems
• Contribute to community engagement, programs and service development initiatives

About You

You're a skilled and empathetic clinician who thrives in a fast-paced, purpose-driven environment. You bring:

• Experience in mental health, counselling or psychosocial support
• Strong capabilities in case management, engagement and risk assessment
• Confidence working with complex or vulnerable client groups
• A collaborative approach and ability to build effective partnerships
• A commitment to evidence-based practice and continuous improvement

Essential Requirements

• One of the following:
• Registered Psychologist, Occupational Therapist or Nurse (AHPRA)
• Social Worker (eligible for AASW membership)
• Clinical Counsellor (PACFA or ACA Level 3/4)
• Ability to obtain:
• Working with Vulnerable People / Working with Children Check
• Baseline Security Clearance
• Must be an Australian Citizen
