Jamia Healthcare AFT Donvale Garden Unit Trust (Donvale Gardens) is seeking a compassionate and motivated Residential Care Officer to support residents' wellbeing and quality of life through care coordination, resident support, wellbeing monitoring, community engagement, and collaboration with families and multidisciplinary care teams.

Task & responsibilities

  • Assess residents' social, emotional, physical, cultural, and support needs and contribute to the development, implementation, and review of individualised care and support plans.
  • Interview residents, family members, and representatives to identify care requirements, personal goals, challenges, preferences, and support needs.
  • Monitor residents' wellbeing, adjustment, independence, and quality of life, and report changes, concerns, and outcomes to relevant care teams.
  • Assist residents to access appropriate health, aged care, welfare, community, advocacy, and support services, including making referrals where required.
  • Liaise with residents, families, healthcare professionals, allied health providers, community organisations, and external service agencies to facilitate coordinated care and support.
  • Promote awareness of available community resources, support services, and social engagement opportunities that enhance residents' wellbeing and independence.
  • Support and encourage residents to participate in recreational, lifestyle, cultural, social, and community activities in accordance with their interests, abilities, and preferences.
  • Advocate for residents' rights, dignity, choice, independence, cultural needs, and overall wellbeing in accordance with aged care standards and organisational policies.
  • Work collaboratively with nursing staff, lifestyle coordinators, allied health professionals, care staff, and management to ensure effective delivery of resident-centred care.
  • Contribute to resident assessments, case reviews, care planning meetings, and wellbeing initiatives to support positive resident outcomes.
  • Identify emerging care and support needs and assist in developing appropriate interventions, support strategies, and service referrals.
  • Maintain accurate case notes, resident records, assessments, reports, and documentation in accordance with organisational, legislative, and regulatory requirements.
  • Assist residents and their families in understanding available support options, services, programs, and community resources relevant to their circumstances.
  • Support the creation of a safe, inclusive, respectful, and supportive residential environment that promotes social connection and quality of life for residents.
  • Participate in quality improvement activities, audits, and service development initiatives aimed at enhancing resident care and wellbeing outcomes.


Qualification & experience

  • Diploma or higher qualification in Community Services, Individual Support, Aged Care, Disability, Health Services, Social Services, or a related field.
  • Minimum 1-year relevant experience in aged care, disability support, residential care, community services, or a similar care environment.
  • Strong communication, documentation, stakeholder engagement, and resident support skills.
  • Ability to work collaboratively within multidisciplinary teams.
  • Understanding of person-centred care principles and aged care standards.
  • Current National Police Check and relevant clearances or willingness to obtain.
