We are seeking a Residential Care Officer to join our team at Lifeline Victoria Pty Ltd. The successful applicant will work closely with residents, families, healthcare professionals, and external service providers to support residents' individual needs, well-being, independence, and social inclusion.

Task & responsibilities

  • Assess residents’ individual needs and develop, implement and review personalised care and support plans. 
  • Provide supervision and structured support to residents living in group housing, including implementing approved care and support plans. 
  • Interview residents and, where appropriate, their families to identify their personal, social, emotional and behavioural support needs. 
  • Plan and deliver educational, recreational, life-skills and community participation programs that promote residents’ independence and wellbeing. 
  • Monitor residents’ health, behaviour and progress, maintain accurate case notes and prepare regular progress reports. 
  • Support residents with personal development, social engagement, community access and with daily routines as part of those plans. 
  • Identify changes in residents’ needs or circumstances and respond appropriately, including escalating concerns in accordance with organisational procedures. 
  • Liaise with families, healthcare professionals, community organisations, welfare agencies and government service providers to coordinate appropriate care and support. 
  • Refer residents to specialist services or external agencies when additional assistance is required. 
  • Assist residents to develop practical living skills, positive relationships and greater independence. 
  • Maintain a safe, respectful and supportive residential environment while following safeguarding, privacy, workplace health and safety, and incident-reporting requirements. 
  • Respond appropriately to challenging behaviours, emergencies and critical incidents in accordance with approved care plans and organisational policies. 
  • Advocate for residents’ rights, dignity, choices and access to appropriate services and community resources. 
  • Participate in care-team meetings and contribute to the ongoing review and improvement of residential support programs.

Qualification & experience

  • Certificate III in Individual Support or higher (Certificate IV or Diploma) preferred
  • Experience supporting people with disability in community and residential settings.
  • Knowledge of relevant practice standards and regulations
  • Strong judgement and management skills
  • The right to work in Australia 
  • Team management skills
  • Strong time management skills

  • Current First Aid and CPR
