An established NDIS provider in Cranbourne South is seeking a caring, capable and community-minded Community Worker to join its team on a full-time basis.
Position Overview:
The Community Worker will facilitate community development initiatives and connect participants with the recreational, health, housing, employment and welfare resources they need to live independently and participate fully in community life. The successful applicant will work closely with NDIS participants, their families, support coordinators and external agencies to assess needs, plan supports, and help people access appropriate services — in line with Life Health Services' NDIS-registered supports including accommodation/tenancy, community participation, daily living and life skills development.

Task & responsibilities
- Assessing clients' needs and planning, developing and implementing educational, training and support programs
- Interviewing clients and assessing the nature and extent of difficulties
- Monitoring and reporting on the progress of clients
- Referring clients to agencies that can provide additional help
- Assessing community need and resources for health, welfare, housing, employment, training and other facilities and services
- Liaising with community groups, welfare agencies, government bodies and private businesses about community issues and promoting awareness of community resources and services
- Supporting families and providing education and care for children and disabled persons in adult service units, group housing and government institutions
- Supervising offenders on probation and parole
- Assisting young people to solve social, emotional and financial problems
- Preparing submissions for funding and resources, and reports to government bodies and other agencies
Qualification & experience
- A relevant qualification in Community Services, Social Work, Disability, or a related field — Diploma level (AQF Level 5) or higher, where applicable
- Previous experience working in community services, disability support, or a related welfare setting
- A current NDIS Worker Screening Check, or eligibility to obtain one
- Current First Aid and CPR certification, or willingness to obtain
- A current Victorian driver's licence (travel between participant homes and community locations required)
- Excellent communication, interpersonal and case-noting skills
- A caring, patient, culturally sensitive and person-centred approach to supporting clients
- Ability to work independently and as part of a multidisciplinary team
- Commitment to upholding client rights, dignity, choice and control
- Knowledge of the NDIS Practice Standards and Quality and Safeguards Framework will be highly regarded
